Please call Vital Health Institute to talk about how your insurance company will work with our office. Our surgical fees are based on the procedures and the complexity of a patient's case. Dr. Cook works at three different hospitals, and most of the time we are able to find one that is contracted with a patient's insurance so that the fees are in-network and the standard co-pay amounts apply. For those patients who are paying 100% of the fees themselves, we have a special program with a local hospital. In addition to the hospital fees, a patient should plan on being billed by both the anesthesiologist and the pathologist. These charges are usually covered as in-network, but if not, you can set up a payment plan. For those who need financing or payment plan options, we work with lending institutions that we can recommend.
